Senior Concierge Services

With your help, Senior Concierge Services recognizes one extraordinary non professional caregiver who is providing care to someone aged 65 years or older in Cuyahoga County with an award each quarter. A volunteer committee reviews the nomination forms and makes the final selection.

You may nominate a caregiver more than once, but, not in the same month.

The caregiver must be a family member or friend providing care for someone 65 years or older, willing to be recognized and have their photo published.

Using this form, please indicate your name, address, phone number, etc. In the comments section, please list the name of the caregiver you are nominating and why you feel this person is worthy of the Caregiver Award.
